ILR And PBS dependent

I am teir 1 general migrant and applying for ILR in december 2013 My wife is also applying but she is not eligible for ILR so applying as PBS dependent how much is fees for her in a same day service? and which form i will use for her?
when booking an appointment for same day service we need to book separate appointment?
Is there any maintenance fund required?
