Help with Application form - Indefinite Leave To Remain

Hi All,
I am inquiring on behalf on my friend as she have difficulty using Internet. Here is the back ground:
My friend was granted two years visa on the bases of marriage to her husband who is a British national. Both of them are in full time employment and her two years visa is due to expire next month. She would need to apply for Indefinite Leave To Remain. Could someone please confirm if this is the right application form:
