I just realised SET (O) form had been changed this week.
My appointment is tomorrow morning.
I'm going to print out using my home printer but I have no photocopier.

Do I need to submit a copy of the forms or just the original will do?

just original is fine, but it is advisable to keep a copy of the completed form for your own record.

Were you ok with the previous version (10/2013) of the form? I have my PEO appointment in a couple of days, and have readied everything using that version.

I'm happy to take my chances with it, but just thought I'll ask since you've applied with the previous version after the effective date for the new version of the forms.
