Help required on my ILR application.....

I am applying under Tier 1 (G) category for ILR next February, under SET O form.
i have a few queries on my application.
1) Is the earliest i can apply for ILR is 02/02/2014?
Valid from of initial stamping: 28/01/09
Date of entry into UK: 08/03/09
Expiry of current leave: 02/03/14
2) On Page 46, point 8 (Give details when your fingerprints were taken):
I have entered the date i gave the finger prints ( "10/02/12"), and "Tier 1 Externsion" as answer in the boxes. #
is this correct?
3) On Page 58, section A "Documents to support your Tier 1 points assessment":
What is the number of documents for this question?
Is it 1, as i am going to give PBS calculator?
OR
Is it 15, for the attribute of age, qualification, earnings, UK experience (i will submit 1 passport, 1 degree certificate, 12 payslips, 1 bank statement)?
4) On Page 19, Section 7.11 (How long have you lived in the UK?): should i enter 4 years and 11 months or 5 years?
5) I have changed 2 companies in my last 5 years of stay.
is there any rule that the break (period between change of companies) in my employment should not be more than a specified number of days?
