Application form for dependent

Hi all,
I am on TG1 and applying for my ilr with 1 dependent (wife), she has been in uk since may 2010 as my dependent therefore also apply for ilr at the same time. I know that for me its gonna be a set0 form, but i am not sure about the dependent whether to put her name on the same form or i'd have to fill another different form for her, sorry if this has been already asked by someone but i could not find anything.
