Absense Letter from Employer - Template ?

Hi Guys,

I need your help in terms of defining the template which I should ask from my employer for absence according to new SET (O) form.

That is my summary for out of the country:

- 2010 – 25 days out of the country for family visit
- 2011 – 20 days out of the country for marriage
- 2012 – 28 days out of the country for family visit
- 2013 - 2014 – 30 Days out of the country for family visit

I cannot get the letter from my previous employer about my family visit but I have been employed throughout from the last 5 years in different companies.

I can be able to get letter from my current employer but they can only confirm my annual holidays that it was paid ?

Please advise the letter format and what option should I tick on the form ?

1. Letter(s) from employer(s) detailing reasons for work-related absences including periods of paid annual leave from the UK?
2. Letter from applicant detailing reasons for absence due to compelling or compassionate reasons
3. Original official documents: (medical/birth/death certificate(s))
4. Letter from travel companies to evidence disruption to travel
5. Other

All of my trips were personal related and not work related however they have mentioned to include period of paid annual leave from UK (so in this case would Option 1 be suitable to tick and produce letter ? )
