FAQ Q3 and Q4 please help

Thanks to everyone for reading this post. If someone can please clarify which is the earliest date I can apply for my ILR I would be extremely grateful.
I got my entry clearance on 07/July/2009
First entry to the UK on 18/August/2009---57 days later
Extension applied on 23/06/2012 valid until 11/09/2014
According to the FAQ Q3 and Q4 the 5 years qualifying day starts from the day one enters the UK in case of entry clearance. So accordingly I should apply 28 days before the grant of EC or before the first entry to the UK.
I'm preparing my documents for the application. But I would need the clarification on the application date to fill the forms and other things properly.
Kindly clarify.
