ILR - Earnings Table - Section 9 - A4 and A5

Hi,
Could someone please help me regarding the SET(O) Earnings table completion part?
I am Tier 1(General) Migrant and applying for ILR next week. I have a question regarding completing the Section 9, question A4 and A5.
I will be providing the earning period 01-Sep-2014 to 31-Aug-2015. In this period I changed my job and started with a new company in Jan 2015. My questions are:
1) For section A4 - Do I need to provide separate start and end dates? that is:
Start: 01-Sep-2014 END: 31-Dec-2014 (ABC company - last date with this company was on 31-Dec-2014))
Start: 01-Jan-2015 END: 31-Aug-2015 (XYZ Company - Started with this company on 05-Jan-2015)
there are four start and end dates option, so I am confused.
2) For Section A5: Do I need to mention ABC and XYZ company details separately in 'Source of Earnings 1' and 'Source of Earnings 2' ? Or is it okay if I put the details of both the companies in 'Source of Earnings 1' and total of both the salaries in this section itself? I somehow think that Source of earnings 2 should be filled in if you have two sources of income at the same time. Please can you clarify this?
