SET (O) Form help

Good day all,
Been reading through the forums in regards SET ( O) form, but could not find an answer.
I have got my ILR on TIER 2 last January 2016 , but my wife as a dependant been refused ILR due to incorrect English Exams. All this been done on SET ( O ) form. Now she passed correct exams I understand I need separate Form Set ( O )
Could you please tell me who is filling :
Section 1 Applicant details? My wife details or mine?
Section 2Dependant. But she is dependant. So what do I fill in this section?

Your wife is applying, she is the MAIN applicant.

so her details in section 1 and the same details Section 2?

No!! She is the MAIN applicant ONLY. Nothing must be put into dependents section unless children are applying with her.

could you please tell me about section 5. It ask about home and finance. She does not have income at all.
it says ' If you are applying in the bereaved partner, we should miss this section?

You need to complete section 5. It is NOT required if a bereaved partner, which your spouse is not.

5.4 in section 5 says ' does a relative or friend of your or your partner, or both of you, regularly give you money' . I do not give any money, we just have joint account, so she has access to all money earned. what shall I put there?

CR 001,
sorry for keep asking the question, but please could answer on the last one?
I have fillet Set ( O ) for my wife, but only things I did not notice is the questions about me as ILR holder ( previoiusly tier 2). Do I have to submit any paperwork, or only photo copy of my ILR and last bank statement will do?
Please suggest
