SET O 6 Years ILR FORM HELP

Good day, all, I was wondering if you guys could help me with my set o form I have a few questions:

I have been granted 6 years 2 x 3 discretionary leave (qualifying for ILR in January 2016)
In section 3 of the set o form, it asks which category- do I have to say other if I am doing it based on the 6 years DLR? and when I state my reasons as to why I am applying for indefinite leave to remain do I say that I've been given and completed 2 x 3 discretionary leave and provide my documents to prove too?
Also, do I need to complete the life in the UK in section 4? - I know I don't have to take the test, however, do I still have to fill out section 4?
