Please help!! 6 YEARS SET O FORM HELP . PLZZZ

Hi all, I need some help. please please please.
I have completed 6 years discretionary leave to remain and now applying for ILR premium service via set o form. On the form, in section 3 i have to state why I am applying and how I am eligible. Should I just state that i have completed 2 x3 discretionary leave, include the dates i applied and was given discretionary leave? is there more i have to include e.g any rules that allow me to qualify etc? Also, when going to the premium service do we have to bring the whole entire form or just relevant sections. These questions may sound basic & silly, but after paying over £2300, i really cannot afford any mistakes.
