Tier 1 to ILR questions

Section 12 has this question.
For the period before you were granted leave as a Tier 1 (General) migrant, documents
showing that you met the relevant requirements of the immigration rules.
What docs need to be enclosed ? Does entry clearance visa stamp on passport do ? Note that it mentions "for the period before you were granted Tier1".

I am applying for ILR SET(O) Tier 1 General 5 year route. Do I need to pay NHS surcharge ?
Did go through previous posts. but confused.

No, not payable for ILR applications.

No, not payable for ILR applications.

No. Its only Tier1 General.

Then I suggest read the question again, concentrate on the 4th, 5th, 6th and 7th words in your first post.
