Documents for ILR under Tier 2 general

Hi All,
I have been on Tier 2 General since 19-7-2012 and changed 2 employers since then.
When will I be eligible to apply for ILR- I guess its 28 days before 5 years. So will it be 22-06-2017?
Also can someone please give the list of documents which I need.

Your 5 year will only be intact if you changed employers within 60 days of a job terminating.

Was never terminated from the Job. Just changed jobs and joined after about week after leaving my previous roles. So I should be good?
Also can someone please provide list of Docs which are required.

Also in application form I cant see anywhere if I need to specify that I have changed the employers and its was less than 60 days.
Also for the holiday(non-work related) absence do you I need letters from employers?

That is part of the rules. The case worker will check that
