Continuous period of five years lawfully in the UK - Tier 2

Hello All,
I was confused with the calculating the dates for continuous period. I have done the calculation as per the document provided in the Calculating continuous period of leave.
I have PEO appointment on 2nd of December. Please let me know if following calculations are correction as I am applying within 28 days of completing 5 years.
Tier -2 Leave starts from 17-12-2012
Year 1
02-December-2017
03-December-2016
Year 2
02-December-2016
03-December-2015
Year 3
02-December-2015
03-December-2014
Year 4
02-December-2014
03-December-2013

Thank you so much for the response. I was thinking to add the list of above mentioned dates in the covering letter and I was wondering if those dates are correct.

Why you need covering letter for ?

I thought covering letter would help Case Worker. I have seen many people on the immigration board suggesting it. Please let me know your views.

Not required at all. The case worker will have all the details of your case. That's one benefit of applying from Tier2 to ILR.
Read the guidance only few docs are required.

Thanks for sharing your views.
There is only one section in Tier 2 Policy guidance which details about Tier 2 to ILR. I have also referred to the Calculating Continuous leave in UK. I also checked the pinned topics about the guidance which also provide the same information. Please let me know if there is any other documents missing?.
Also in SET O application form, they have asked to provide the evidence of leave before Tier 2 Leave - " For the period before you were granted leave as a Tier 2 migrant, document showing you met all relevant requirements of the immigration rules. I have been in Tier 2 General all 5 years before that I was in PSW - then Tier 4.
What kind of evidence if required to show. I can see some members suggested, it is not required for Tier 2, but Is there any reference to this? Please let me know your views.
