ILR after 6 years DLR with 2 years spent conditional discharge

Hi Guys,

I your help.
My 6 years discretionary leave is due to expire on 11 March 2018.

The first DLR was given Feb 2012
The Second DLR was given 11 March 2015, which will expire 11 March 2018.

The first DLR was given due to an unspent 2 years conditional discharge in December 2011.

Please my questions are

1) Is it OK to apply for the ILR in person, as I've already booked a place and also since my conviction has been spent.
2) Is the SET(O) form the correct one since my last 3 years DLR was on FLR(O).

Please I need your advice.

Yes you may apply In person, and yes SET(O) is thr correct form.

Please I have one more question.

Should I declare the 3 points I had about 5 years ago on my licence, which on the government website it's says I have 0 points and also PCN charge I received in the last one year on the SET(O) form under the conviction section?
