PBS Dependant to ILR

Hello all,
I just got my ILR granted in early January 2018 from 5 years Tier (2) General Route.
My wife and son (born outside UK) will be eligible to apply ILR in May 2018 using Tier-2 dependant route.
My questions are:
(1) My wife works part time and got her own NI number. Does she need to complete question 5.3 in SET (O) form about income and include her payslips? or we can skip it altogether.
(2) Does she need to present her own bank statements or mine?
(3) Do we need to complete section 12.l to list the marriage certificate?
