ILR based on 5 years of Tier2

Hi,
I and my wife has been on Tier 2 for last 5 years and both are now eligible to apply for ILR.
She entered the country as a Tier 1 PSW dependent but changed to Tier 2 General visa along wih me and since then has been a dependent on my Tier 2 applications and extensions.
My wife has been a dependent throughout and have taken taken any job or any other visa.
My question is that do I need to fill in two separate Set (o) forms for me and my wife mentioning each as main applicant in each of the forms and filling up the main applicant sections only or can I submit only one set (0)application form mentioning my wife as dependent and filling dependent information for her?

Kindly advise.

Your post is unclear. Is she a Tier 2 (General) holder or a PBS family member?

If the latter, then you may include her in your ILR application.
Family members (dependants)
Your partner and children under 18 can apply on the same form as you if they’re eligible.
Else, separate applications.

Thanks Vinni. Sorry for the confusion. She is a tier 2 dependent and does not have a tier 2 general visa of her own. So, I guess it will be one form.
