I'm currently on tier1 entrepreneur extension visa and due to expire on 28/01/2019, eligible to apply for both SET (O) and SET (LR) in second week January 2019. Due to long delay, rejections and long wait decided to go for SET (LR) same day service

I've 2 dependents on me my wife and 8 months old child born in uk

what application form I need to submit for the dependents?

what are the required documents to submit for the dependents ?

need to show any proof of income or my wife need any English language test certificate ?

Planning to apply SET (LR) same day service just for myself then after I will apply for dependents later on, just in case if my application not decided on same day how can I apply for my dependents ?

planning to book a slot around 2nd week of January before 2 weeks of visa expiry date

Please help me any of the seniors

Read the flr m form and guidance notes. It tells you what is required.

Yes you have to meet the financial requirements. How many children do you have? Wife needs English A1 test.

How long have your dependents been in the UK?

FLR M will reset their clock to zero and a new 5 year (2 x 2.5 year visas).

My payslip salary is £1350 per month and taking £2000 as dividend vouchers

I don't know how to prove £22,400 income requirement per dependent with one child

You should read FLR(M) form and then ask questions if you are unclear.

Yes you can, two separate appointments. However, if your ILR fails or is held for further queries, her FLR(M) will also fail.

I have been through FRM form and unclear on financial requirements.

I'm unsure on what documents need to submit for self employment?

Guidance is more relevant to employment not for self employment it says "letter from employer, pay slips and proof of income and bank statements"

If you don't mine please provide me the list of financial documents to provide for financial/income satisfaction for self employed.

I worried about taking the salary from company as £1350 per month and £2000 as dividends

guidance states income should be £22,400 for dependent with one child. I don't know how to prove this.

Registration document and permanent residence

Unfortunately I have only registration certificate document it makes any problem to claim the points

The guideline says
(iv) if the employee is an EEA national, a UK registration certificate/permanent residence document;

Where did you find the Registration certificate and permanent residence
