ILR set (O) enquiry

hi,
i am applying for ILR as a dependent of tier 1 general applicant on PBS.
i have completed my 5 years as a dependent in september.
My husband is settled and has a British citizenship
1-I have recently seen a SET(O) form online. Could i fill that?
2 Regarding Finances there is a question if i have ever claimed any BENEFIT
I have a child who is a British citizen and we get child benefit,Do i declare that as well?
3-Is there any financial requirement?
4-What are the supporting documents i have to provide?
ALL the utility bills etc from (5 yr period of my stay in uk)
5- what is the total Number of days absence from UK?
