I have a premium service center appointment booked. I used my wife's debit card for booking the appointment so in the payment details of the SET(0) paper application form, there is my wife's name and signature.

Should I submit my wife's bank statements (She is not applying with me) as additional proof?
Is this a cause of concern? Should I take her debit card along when I visit the premium center?

There is no requirement to complete the payment page of the paper form if you have paid online already.
