Help to find out correct application for

Hi I have a question about my application. I filled in the previous set m form by downloading it and filling it in but just realised yesterday there is a new online application form from 1st November.

I booked a premium appointment for 16th November and still have the completed old form. Back in October I even completed my settlement check service with the previous form.

Now I’m conserned if I need to apply online or if I can still use my old form. I’ve tried contacting the premium centre to check but no luck. I don’t want to turn up with the wrong form!?! Then again I don’t want to fill in the form again online if I can still use the previous one.

Any advise would be much appreciated.
