Advice on Tier1 Ent. Extension & ILR premium service

Dear Marcnath and other respected Members,
I am going to Apply Tier1 (Entre) Extension end of this months via post with my wife as dependent. Could you guys please advice on the below points,
1. I will be eligible for ILR(10 years-LR)in January 2019. is it possible to Vary my Tier 1 Extension at Premium center to ILR (LR) or do I need to post ILR application first with variation cover letter and then upgrade to ILR?
2. What will be the dependent's situation, stay same as Tier 1 (Ent) extension dependent's application, until my ILR decided?
3. and thirdly, how much do I need to pay health surcharge ( Tier 1 (Extension) application ) for dependent and myself? I mean am going to vary application in around less than 2 months,. ?

You can vary to ILR but PSCs will all close by the end of the month. You may apply under SET(LR) online and use
the UKCVAS services to submit your documents in person. You will not get a same day decision
2. What will be the dependent's situation, stay same as Tier 1 (Ent) extension dependent's application, until my ILR decided?
They should switch to spouse route FLR(M) if you choose to apply via long residence. Their ILR clock will reset to zero. FLR(M) route also has English language and financial requirements to satisfy.
3. and thirdly, how much do I need to pay health surcharge ( Tier 1 (Extension) application ) for dependent and myself? I mean am going to vary application in around less than 2 months,. ?
They cannot continue under PBS if you choose to apply for ILR under long residence.
They must vary to FLR(M)
There is NO IHS to pay for ILR but the IHS fee under FLR(M) remains £200 per year per person but this
may be increased soon.

can you be kind enough to clear the below points,
1. how to proceed variation with new service. I mean, I just need to write a cover letter with SET(LR) application and approach UKCVAS to upload/ submit documents? is there something else I need to do?
3. My Tier1 (E) extension is due in this month, But SET (LR)Application variation will be in January. Now I have to pay Tier1 E Extension Application IHS Fee, right. Do I need to pay for 2 years or 1 year?

1. It is a new service - that level of details are not clear. But if there is no specific option, a cover letter should still work
2. When you do your IHS, it will tell you how much you need to pay. It has been 3 years so far. I assume it will be the same.
