Could you please advise if there's any requirement to fill-up the set (o) forms manually when I submitted my dependents' online application (separately) in Premium Service?

Also, I don't see Set (O) forms available online which used to be there a few months back? cheers!

Hello,

As far as I am aware, there is no the manual form any more. Online application only.

You only have the online form, link below. No requirement to complete a paper form too.
