applying for ILR? eligibility, documents questions and new VCAS system

Hi,
I am planning to make my application for UK ILR soon. I worked on a Tier 2 General visa for my previous employed from October 2013 till Nov. 2014, and since then have worked for my current employer. My current T2 BRP expires in July 2019.
Given that I have been on Tier 2 BRP's since Oct. 2013, I believe I am now eligible (5 years) to apply for ILR. I recently passed the Life in the UK test as well.
Regarding the ILR application, I have a few questions:

Regarding the ILR application, I have a few questions:
- How to apply? Is the application completely online now, and not paper-based anymore.
- For these online applications, can we still later book a priority or premium service as I read that standard ILR applications can take upto 6 months in some cases?
- Also, if I apply in Dec. 2018 or Jan. 2019, will I use the old priority/ premium service method or the new VCAS system rolled out on Nov. 29th, 2018?
- For ILR application after 5 years on T2 BRP, it's the SET(O) form to be filled, unless I'm mistaken?
- SET(O) ILR fees clarification: it's £2389 as of now? plus approx. £600 if applying with premium appointment (and no health surcharge for ILR), unless I've got it wrong??
- Documents to provide: 1) passport, 2) old BRP, 3) Life in the UK test pass certificate, 4) proof of employment (need to ask employer for letter confirming this, any templates available for the required wording?), 5) proof of savings (is there a minimum balance requirement?), 6) English language proof (I completed my Masters in the UK, so should be exempt from this, as I was for past Tier 2 General applications, unless I'm mistaken?), 7) appropriate salary (My current CoS is for a PhD level role, so I believe I am exempt from this as well, though I do meet the salary threshold as well).
