lost life in UK pass notification

Hi All,
I have lost my life in UK pass notification. When i called Home Office the person said that i can write a letter stating the date of exam , centre where i gave exam . Please can someone confirm if they were able to get application approved without submitting the pass notification. Or would you recommend to give exam again before applying.
