ILR due and stuck abroad

Hi,
My spouse is due to fill ILR form very soon. But she got stuck in her home country due to an unfortunate medical condition. It might be possible that she may not be able to come to UK before her current VISA expires. What are the options now ?

Can she file ILR in her home country? I think not.
If she is not able to come to UK before her visa expires, what visa she need to file then ? how does it affect her ILR ?

Note: her medical condition is already known to NHS here.

Apply for a visa to come back or return if possible before her visa expires.

No. You cannot apply for ILR outside the UK at all.
If she is not able to come to UK before her visa expires, what visa she need to file then ? how does it affect her ILR ?
Presumable the same visa she has now, you don't state what that is, but assume it is a spouse visa. Likely her clock might get reset to zero. Note also that the IHS fees have doubled recently.

yes, She is currently on spouse visa FLR(M) . (form to extend stay in the UK as the partner).

So if she does not return on time, she will have to file the VISA that was filed the very first time ?

Correct yes and meet all the requirements again, pay £1523 visa fee PLUS £1000 Immigration health Surcharge.
