Since your extension was approved in May 2018 and if you apply in April 2019, you will be able to claim all employment 12 months immediately from the date of ILR application as you fall under the exception rule. Note: If you apply after 6th April 2019, you need 2 FT JOBS that existed 12 months each not 2 employees. If you apply before 6th April 2019, then all you need is 104 weeks of FT employment
3) Employment I will send FPS, RTI and copy of passports
yes
4) Do I need to show start dates of employment? if yes how to show start date for an employee working since 2016? send old FPS?
no

no need
You are eligible to apply for ILR since late January 2019. Given, if you had employment during your waiting time and lets say you apply on 25th Feb 2019, you can claim all employment from 25th Feb 2018, so in that case you dont have to wait till April. Just clarifying so you know that you can benefit from the exception rule.

You dont need to send English proof if u have already qualified for it at extension time. However it does show up in document checklist which makes it controversial. I think you can refer to extension guide for not sending which is better than sending expired. And it will be unnecessary to sit in exam again provided it is not required.

Once you have presented it during the initial and extension applications you don’t need to present it again. You will just mention it has been met in previous applications

What day in May 2018 has it been approved? if it's say 15/May/2018, then the risk is that the employment since 15/May/2018 will only be accepted (from 01/May/2018 to 15/May/2018 may be ignored). I'm not quite sure about this and hope Marcnath can kindly advise

The OP falls under the exception rule. The above what you said does not apply to him. He can claim for job creation 12 months immediately before date of ILR application. So what you said is not valid. If OP applies in April 2019, he can claim from April 2018.
