ILR for Wife (5 year route on FLR-M)

I obtained BC through ILR via 10 year long residence route. My wife's spouse visa (FLRM) was granted from May 2014. So, i understand she becomes eligible for ILR in May 2019 (under 5 year route). I have the following queries please:
1) What language test does she need to pass, A1 or B1? She passed A1 from one of recognised test centre (in 2014) which was used for her FLRM leave application within UK.
2) Which form to use?
3) Can she apply 28 days before her 5 years are complete?
4) Which method of application is preferred (post or online, fees and timelines)
5) Evidence required to be submitted?

Just further clarification re the form, she was a WP dependent from June 2011, then Tier 2 Dependent from Apr 2013, then spouse of Settled person from May 2014. So is it SET M or SET O she need to fill please?

Based on this, what will be the answer to the following on the form please:
Were you originally granted leave on the basis of an application made before 9 July 2012 as a partner on the family route? (Required)
Your original leave to remain would have been for 2 years. You cannot have had an application considered, granted or refused on another route since that time.

She was not granted a spouse settlement visa prior to 9th July 2012. So the answer would be NO.
As you got ILR based on 10 years long residence, she had no option but to change to FLR(M), which reset her ILR clock to zero.

thanks. one more please. Date I obtained citizenship? Is this the date of ceremony or date of receipt of approval of naturalisation application?

Date of ceremony as you are only British at this point.

thanks. And another one, 'Could you and your partner live together outside the UK if necessary?', i know it relates to the individual. From spouse's perspective, she wants to live with the husband forever.
What is the correct answer to this.
