ILR New Process

Hello. I am also planning to apply for ILR this month and have some queries for the Tier 2 General route Set (O)
1. What all documents do we need to submit?
2. Once we fill the application, do we immediately need to upload the documents or we get some time?
3. Can we chose the day of appointment post paying for the application?
4. What document should be provided against the point which says "for the period before you were granted leave as tier 2 migrant, docs showing that you met the relevant requirements of the immigration rules"
