SET(M) questions for the application (changed name)

Hi,
I have some difficulties to figure out what to do with my last name for my SET M application.
Supposing that my original last name was 'A', my husband's last name is 'B',
-> Entered with the previous passport under surname 'A' on FLR(M) in August 2014
-> Got married to British citizen in 2016
-> Extended FLR(M) in Feb 2017 : granted as name 'A'
-> Current BRP is named 'A'
-> Changed my last name to 'B' for my bank account and driving licence in 2017-2018
-> Changed my last name to 'B' for my application of register traveller(UK home office issued) in 2017-2018
-> Renewed my passport and added my husband's last name on Surname section like "A (Spouse of 'B') in 2019.
Sorry, it sounds a bit weird but it happened.;;;;
Could you help me ,please?
1. which surname can I use as applicant's name on my ILR application?
Should use 'B' and add 'A' info to other name section?
2. I am also a UK register traveller. used surname 'B' when I applied it to Home Office and granted. In that case, should I answer "Yes" from the question ' Have you ever entered the UK using this name 'B' ?'
