I applied for my ILR based on tier 1 ent last year and I am still waiting to hear back. I applied for her extension but haven't received it yet. So

Now, I have applied for my wife's ilr as her visa was issued on 24th June 2014 and I applied on 6th July 2019.

I needed some information about the documents that I need to upload

I am uploading the passport, Cohabitation proof, life in the uk, English language test results

Is there any thing that I need to add to it?

Can anyone please help?
I have submitted the application online now. But I am unsure about which consent form do we need to sign? What we have done is

Part 1 - signed by my wife as she is the main applicant
Part 2 - signed my myself ONLY as we have some documents in our joint names
Part 3-didn't submit as thought it was not required.

Is it correct? Or do I need to change something
