Tier2 to ILR absence letter to whom and listing absence date of out of country only

First of all, thanks to everyone viewing my questions and supporting me.
I have two questions.
Based on the template of absence letter from our website.
indefinite-leave-to-remain/employer-let ... l#p1052927
Should I use
UK Visas and Immigration
Home Office as To whom
or should my employer use To
whom it may concern for the absence letter?
The second question is should my employer only list the paid annual leaves of out of UK which means does not need to list sickness leave and Paternity leave? I do not have paid business travel. Based on

1) Any. Does not matter.
2) Your employer just needs to confirm that your annual leave outside UK was authorised.

Thanks for your quick reply.
One more question is my flight date is Saturday or Sunday, but my paid annual leave starts from the next Monday.
Therefore, my paid annual leave date is late than my flight date. Do I need to explain this in my application? Or I ask HR to modify my paid annual leave to start from my flight date?
Do I need a cover letter for application? Sorry for this silly question.

One more question in the absence date of each annual leave in the letter
Can I only list start date and end date of paid annual leave without total days of each annual leave? for example below. If total days of each annual leave is a mandatory field, Do I need to explain the weekend and bank holidays of each annual leave?

How to sign consent form of my case

Thanks to everyone for dive into my questions.
I am applying for ILR from Tier2 5 years route.
My application is with one dependent(my wife). Finally, we are at the last stage, which downloads the consent form and signs it.
I don't know how to sign it? It has 3 parts.
Should I print 2 copies of the consent form, I sign one copy and my wife sign another copy?
Or should I print 1 copy and we both sign it.
Should we sign 3 parts or only the first part?

Can you help me in term of consent form?
My case is I am holding a Tier 2 visa as ILR main applicant, my wife is my Tier 2 dependent visa. We both have a job. We do not have financial support for our ILR application.
I do not understand this consent form. Should I sign part 1, my wife sign part 2, part 3 is N/A?
Or we both need sign part 1, and part 2?

Applicant signs part 1. You (sponsor) will sign part 2 for your wife. Leave Part 3 blank as its N/A.
