ILR Application SET M wrong form used

Hi,
I have applied IL on 11th of april using set M. visa expired 16th of april. last week (may 12) i have received mail from home office saying that we use the wrong form.
pls help on what to do. we should have used set o.
I am doing my new application now but Im worried because my visa has expired- does this mean I will get refused with my new application.
need your help

i am the dependent of my husband who is now settled, tier 2 skilled worker.

You need to complete form Set O

thanks for your response.
I am currently completing set O, im a bit worried as there is no question for me to enter previous address in the last 5 yurs, is this not necessary? we just moved to our current house and we are living here for only 8 mos. but we are living in the UK for 5 yrs now in the previous house that we rented, just worried as I do not have options to enter those address. pls enlighten me. thanbks again

Not required. Simply complete was hat is asked for.

Fill out the form as it is. How you 'feel' should not be important
