Employed AND self employed: Form SET (O) - ILR question

Hi,
Suppose someone is both employed and self employed.
But the major income is through employed (as employee, doing a permanent job).
1. If we get a lesser amount through self employment such as consultancy, should we add that too in Form Set O?
Or is the employee (main) salary enough?
2. If both incomes come into the same bank account, then what should we do?
3. In the case that both incomes come into same bank account, is it okay to leave out mention of the self-employment, and enter only the employed salary in Form Set O?
4. If you haven't completed the tax return for year 09-10 (as last date is 31 Jan 2011), but have only 08-09 tax return, is that okay in case you mention the self employment too?
