SET (O) ILR application to be made in NOV 11

Few questions i have got.
I got my WP in 2006 but changed to HSMP in early 2009 , and i have to apply ILR in Nov/Dec 2011.
Now the questions.
1- Can i apply 28 days before actual expiry date on my visa
2. Do i need to send payslips for all 60 months.
3. Part of the period i was self employed what do I need to send for that.
4. How will they calculate my points. i.e will i have to show income of 35000 for last five years or just 12 months before date of application.
5. My wife will apply with me for ILR , but her stay in UK is less than 2 years before she apply for ILR , can she apply withe me or she has to pass a specific time period before eligible for ILR.
6. Can i show my self employment income for last 12 months or just last 3 months or 6 months.
