SET (O) for dependent

Hi
iam applying ILR as a dependent along with my wife. we are applying joint form SET(O). right now we are tier 1 general visa.
I have proof staying with my wife for 2 years
and I passed life in the UK test.
In SET (O) form , there is only one page for dependent ( Section 2 ) to fill , Is there is any other form I need to added along with this SET (o) for dependent. or section2 is sufficent.
Can you please explain what is SET ( M) ?
