URGENT!!PEO APP 28th. WHAT FORM TO CHOOSE FOR WIFE!PLZREAD

Hi,

i have an appointment for ILR @PEO Solihull this Thursday, 28th July.
Have a typical case for my wife and kid on choosing the correct application form.
My wife had been living with me for the past 4.5 years however, she had not lived in the UK for 11 months from June 2010 till 2011 and since May 2011 she has been living with me. While in the absence she went to her home country and delivered a baby girl. now the question is what application form shall i choose for my wife and kid?
Shall i Just include them in my SET(O) form? or is it better to have a SET(M) form for my wife and kind?

please please advise! I have all the documents with me except for this confusion which is literally driving me nuts.

Mar 2007 - wp dependant
Aug 2007 - HSMP Dependant
Aug 2009 - Tier-1 General Dependant


she had continuous visa since March 2007 all visas were extended in the UK along with mine.

Many thanks for confirmation. That was what I was advised by CS while booking the appo over the phone too. But do not know really whether i should disclose/hide the fact of my wife was not with me for 11 months. any advise please?
there are no questions in SET(O) form that specifically ask for the Spouse's leaving entry dates but wondering if they found out they reject her applicatin on the basis of wrong application selection (SET(O) instead of SET(M)) but again i am completely new to this and need you expert's advise.
