I am here in UK on 17th Sept 2006 and changed to Tier1 on Sept 2009.
Will be completing my Five years stay on 16th Sept 2011 applied 20 days ahead.

The following are the documents i submitted and my ILR is approved

1. Application form + photographs
2. 12 months Payslips
3. 12 months Bank statements
4. LIUK certificates
5. Letter from employer
6. HMRC five years statement
7. P60's recent 3 years 2008-09, 2009-10, 2010-11
I donot have the old ones.
8. Cohabitation documents
a. 2009, 2010, 2011 council tax bills
b. Letter from dentist (with address, registered date, appointments)
c. Voter registration letter (2010, 2011)

Point i learned is:
I was worried about the proofs for my work permit time/Tier 2 (total 4 years) time.
The following documents made me get the ILR.
1. I had only 2 years P60's confirming old employer
2. Relieving letter which had starting date, leaving date.
3. HMRC statement

They confirmed with me whether i have any P60 from that employer, So i hope we need to show
some kind of proof for each employer you worked for.

They did not ask me payslip or bank statement proofs for previous employer (Work permit proof)
even though i donot have all the P60's from that employer
and the two years P60's from my old employer were sufficient.

8. Cohabitation documents
a. 2009, 2010, 2011 council tax bills
b. Letter from dentist (with address, registered date, appointments)
c. Voter registration letter (2010, 2011)

I ticked YES for the Continuous Residence Assessment (as i am just using my allowed 28 days ahead option)

My income increased from my initial tier1 so no problem with points.
