I need to ask both of our passports back (EEA1 and EEA2) I'm a bit confused about how to fill the form...seems a bit complicate...
please help..my mother in law is really ill and I need to get back to italy before something happens

What kind of help? Which bit?

Like this option.. Home office (HO) reference or fee reference* – Nationality only...
I left it blank in the end...

If you have received a CoA with a case ID then enter it there. If you do not have one then yes, leave blank.

I just sent them a first class mail ...still no answer...
