EEA residence card holder husband visa application

I'm EEA resident card (5 years) holder (dad is italian nation but i'm not) living with my family in Uk. I'm student (20 years old) and want to bring my husband here in UK who's currently living in Pakistan.we got married 2 years ago in December 2012.
Can someone please help me with what forms to fill and what documents will be needed?
