Question about EEA/PR form EEA residence documentation

Hello,

I was hoping someone would be able to shine some light on this:

I'm a bit confused about the following section and what it's actually asking me to provide

Current or previous EEA residence documentation p.29
5.5 Do you currently hold, or have you previously been issued with, any EEA residence documentation listed in table below?
Yes No
If yes, please provide details below. Continue on a separate sheet if necessary.
Document Date of issue Date of expiry Document reference number
EEA family permit
Registration certificate
Residence card
Document certifying permanent residence
Permanent residence card
Derivative residence card*

I'm a polish citizen and I have a Polish passport, as well as a Polish ID card - it is the ID card that needs to be listed here and would that be under 'residence card'? Or are they asking for something different entirely here? I'm not familiar with any of the listed documents.

Any help would be greatly appreciated

An ID card is your travel document and has nothing to do with the documentation listed. What they mean is documentation from the HO. If the listed documents do not ring a bell, chances are you have been issued with any of them.

I have the same question, however I'm with Bulgarian nationality and have had Yellow and Blue certificates to allow me to work back in the day. Can that be counted towards Registration certificate?
