Italian married BC living there apply for EEA PR

I am Italian living uk over 10 years,now married to a British citizen for over 5years I am applying for residency card on my 5 years in the uk as required for PR .
I am a deaf register disable person on disability for the last eight years .
What section in pr forms am I to complete,the home office will not be against my application for pr due to my deafness and claiming?

Disability does not count for or against PR applications.

To apply for PR, you need to have exercised treaty rights for five continuous years by doing any combination of the following;
a) working
b) seeking work
c) studying - only counts if you have Comprehensive Sickness Insurance or a non-UK EHIC Card
d) self-sufficient (such as by your husband supporting you) - only counts if you have Comprehensive Sickness Insurance.

So, which treaty rights have you exercised and when?
