Help With EEA PR Form For EEA Family Member

Direct family member living in UK since 08/08/2011 soon 5 years
with right to reside in the UK and habitually resident as family member of EEA national.Have EEA pension form EU country and guarantee pension credit since 20/09/2012 back dated from 04/05/2012 did live with daughter for 3 years last two years living in Housing association flat claiming housing benefit and council tax reduction daughter still in uk working 'qualified' person - worker living and working in uk since 31/05/2005 with out breaks apart of holidays
How to apply for PR do I have to prove that family member is still dependent on daughter that she look after her help her and do shopping etc
Is there any problem if they don't live at the same household, but been living together before for 3 years

Can her PR application form be declined?

New form seems to be more complicated than old from EEA3 so many questions

Anyone has any idea?

I sense this is for a parent.

Yes, proof of dependency on sponsor is required .
And proof that sponsor has PR or is a qualified person (etc etc).

No, cohabitation is not mandatory under EU law,

Yes a PR application can always be declined for a whole host of reasons.
Good documentary supporting evidence to back up the case will go a long way.

Yes it is for my parent

yes she is dependent on me i do help with everyday task at home and I do also do shopping but how I can prove it do buy things for my mother at the same time when I do mine shopping I don't keep all receipts I didn't think I will need those yes i buy clothes and other things too I cook and prepare food I do help with cleaning etc ect I do give emotional support take her out when we have more time also take her to all doctors and hospital appointments? Don't have idea how I can prove it that I support I don't put money in her bank?

yes I'm qualified person been working in uk for 14 years and have all proofs like P60 and payslips contracts only had time off for holidays

Have no clue what I can do Please need some help or advice
