PR card - how to fill in application

I will try and keep it brief.
Italian national. Have a passport recently issued. Previously used ID card to travel. Lived here in the UK with a verifiable address history since November 2010.
I have always lived with my partner, British national. Initially he supported me financially. I've continuously held throughout these years a valid EHIC issued from Italy.
I registered as self-employed in April 2013 but it was just some freelancing work with little income so negligible I'd say. Started working part time in May this year (first 15, now 20 hours, earning above minimum wage) so I'm a regular employee and still also self-employed but not making any money at the moment from self-employment.
How do I fill in the PR application? Do I apply as just self-sufficient? Is a valid EHIC then acceptable as a form of health insurance? What exactly do I attach as evidence that my partner has been more than able to support me financially all these years? How many bank statements etc? I can't find detailed info like this anywhere. It's very confusing. Thanks in advance.

Unlikely you are a worker or selfemployed. Sounds like you are selfsufficient.

Seems like EHIC is acceptable to just certify PR.
Will attach payslips (guess one per year is enough to prove partner's continuous full time employment and salary, proofs of address (like joint council tax bills, tenancy agreements etc) to prove we have been cohabiting the whole time since we don't have a joint bank account, in order to prove I have had access to his funds.
I applied for the NI number in December 2010, so have a letter confirming that. Guess that is sufficient to prove the start of my permanent residence. Is it worth submitting a SAR to the UKVI? Have travelled previously on an ID card.
Last time I went back to Italy in June this year I used my passport but there is nothing stamped inside. They just looked at it, so don't see any difference with an ID card.
Does everything I attach as evidence need to be original or is it just ID that has to be original? If they want only originals for everything, as it seems, how do I go about asking for them back after they receive the application, since I read it can take months for them to process it. I'd like to have them back in case I need them for something else. Do I provide them with a pre-paid envelope to post them back to me and politely attach a note to request them back? Thanks in advance.
One last thing: if I decide to then apply for citizenship as technically in November 2016 it will have been 6 years in total that I've been here, does my PR start from when I receive the certification back?
