absences from the uk(permanent residence application)

Hi I've lived in the UK for 11 years now, i've decided to apply for a permanent residence card in order to try and get british citizenship.
On the application form it asks for any absences from the uk since the day i've entered the country.
Does that mean i have to remember all the exact dates that i've went on holiday etc.. or does the last 5 years would be enough?
Is there anyone i could contact regarding this issue, or has anyone got a clue what to do in the situation?

Are you an EU citizen?

Which form are you looking at?

Yes I'm an EU citizen(polish) applying for a permanent residence card
I've lived in the uk most of my life, and this application is really confusing me!

Fill in form reporting absences as best you can; use month/year if some specific dates elude you.
