EEA permanent residence - online application question

Hi, does anyone know if I can apply for my child as well, using the online application form?

https://visas-immigration.service.gov.uk/product/eea-pr

The paper form does include questions for dependent children, right? However, the online form doesn't seem to give me that option, is it possible to make a separate online application for my child?

Also, I have 2 more questions re: my own application:
- benefits - there is a question on that, we only got child benefit, but a few years ago I also got some child tax credits, for a very limited period of time. Do I need to list those old tax credits or just the current child benefit?
- travel - do I have to declare all personal and business trips for the last 5 years? I don't really have records of all business trips.

As per Beta website:
Family members should download and fill in the paper form.
Were you working when receiving benefits or were you in another category of qualified person?

The application is not just about last 5 years (if you've lived in UK longer than 5 years).

Declare month/year of trips if you can't recall every date.

Yes I always worked full-time, and in Oct 2011 I got a blue card (as EEA unmarried partner of EEA), but in Jan 2012 I got a highly skilled blue card (in my own right).

Jun 2011 (moved to UK) to Sep 2011 - I lived in the UK but worked and paid taxes in another EU country. My partner worked FT in the UK, he had one of those 'pink' work cards.
Oct 2011 until present - worked full-time, no employment gaps (one employer only).

Benefits - child benefit since Oct 2011. Also in 2012 I applied for some child/work tax credits - but very soon those were reduced to nil as my income was too high.

No issue with those minimal benefits as you were a worker.
