Is it really necessary to have a Insurance when applying?
Me and my wife (she is the EEA citizen) have been here since 2010 and we used to have insurance up to 2012 but since then we started paying National Insurance after registering for self assessment / self employment.

How much of a problem can this be if we do not provide this?

OK, upon reading the instructions more carefully and not freaking out that I will be kicked out of the country I can see that comprehensive sickness insurance is a requirement if you are applying as self sufficient / student.
I guess that is good news.

I am applying for EEA PR soon as the spouse of a Swiss national - We were advised to actually apply together, since she doesn't hold a RC and brexit and everything.
So we are going that route and doing the application under her name and adding me as the family member.

My question is if she is the applicant and is a EEA national do we still need a Sponsor?

And since I am asking questions, I've seen some people mentioning the use of an old EEA4 form as it was much simpler than the current one! What is the general thoughts about this here... would the case worker hold anything against our application for using an outdated form?

No need for a sponsor.

Currently the current form is not a mandatory legal requirement.
People (members) can and do use earlier (simpler) forms. No penalties.

This may change if/when recently announced UK Regulations come into force. This new approach is not in compliance with EU law but in meantime apply soon if you plan to use an earlier form or letter.
