I see home office reference number and COHID but no appeal number/port reference number on refusal letter.

Hi guys,
How do you appeal a eea refusal.
I have found the online form iaft5 but dont know where A N reference number and port reference is. Can only see home office reference number on refusal letter.
I only received refusal letter and reasons for refusal.
If possible moderators could you merge my other thread with this since i couldnt edit title. Thanks and apologies for the inconvenience.

COHID, and Home Office Reference if available, are all that are needed for in country appeal.

Unable to appeal.
Keep getting error in section 2.

Does the 14 calendar days start from date of refusal or date of service?
