EEA(PR) Form applicant plus 4 family members

Hi,
my husband is the applicant in our EEA(PR) form.
There will be me as a spouse added in section 11,
one child under 21 in section 12
two children over 21 in section 13
But sections 11-13 seem to point to a sponsor (in section 2).
For us my husband would be the sponsor. Should we fill my husband under Applicant Section 1 and then again as Sponsor in section 2?
OR is it logic, that wife and children of applicant depend on applicant, so we can leave section 2 sponsor empty?
