EEA(PR) self sufficient route

Dear members please advice me, how to meet EEA(PR) application requirements in self sufficient route.
I have private health insurance for the last five years and bank statements for the last five years on my name.
I go through the guidance notes in which home office asked comprehensive insurance and bank statements for the appropriate period plus one piece of document to proof Residence in the uk.
Now I don't have any other piece of documents except bank statements.
Is this acceptable for PR application if I use these bank statements as a proof of Residence and as a proof of funds?
And what does mean for appropriate period?
Your advice needed if some body can help out me.

Are you saying that you don't have a Tenancy Agreement nor Utility bills under your name?

Yes
Only can provide bank statements.
Is it acceptable for pr application if I submit private health insurance and bank statements only?
